I have a few more items that require attention before our trip. ALL going on this trip need to turn in ALL of these documents. Please complete and turn in by next Tuesday if possible.<br> 1)Rafting Waiver Form. This was handed out at the trip meeting on June 4. We have extra copies at the scout lodge.<br> 2)Troop 17 Release Form. This is posted on the website with the gear list and we have extra copies at the scout lodge.<br> 3)BSA Medical Form. Instructions and form links below.<br> https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_instruction.pdf <br><br> https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf <br><br> The rafting waiver form and the Troop 17 release form as well as sections A and B from the health form can be filled out by an adult/parent.<br> Section C from the health form requires a licensed physician (MD, DO), nurse practitioner or physician assistant. If your are unable to get in to see your physician the local minor emergency centers will generally complete the physical for around $25.<br><br> I have a master list that tracks what each individual signed up for this trip needs to turn in.
