If your last name sounds like Vallee, Upchurch, Plavidal (I left a vm for mom), OBrien, Noeth, or Brogdon, we do not have an up to date medical form for you. Please go to CareNow or your own favorite local physician with the BSA medical form (http://www.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf) and get a physical.
