The link to the medical form for Summer Camp is available at https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf Please be sure to download the form and save it locally to fill it out. This requires a physicians signature and the shot records filled out. BSA policy prohibits scanning the document so please send us a paper copy for our binder.
