The link to the medical form for Summer Camp is available at https://filestore.scouting.org/filestore/HealthSafety/pdf/680-001_ABC.pdf Please be sure to download the form and save it locally to fill it out. This requires a physicians signature and the shot records filled out (please complete the box on the form - do not write "attached" - if you dont fill it out, Mr. Turner will have to fill them all out). The form must be signed by a physician within the last 12 months. Care Now and other local clinics can complete physicals for around $25 if you cannot get to your physician. Please bring the completed forms on Tuesday. Only 17 days until The Big Adventure!
